Job Description

Elevate your career as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members. This per diem opportunity offers flexible scheduling and the freedom to design your own workweekshifts are available Monday through Sunday to best fit your availability. Serving various communities within Sharp County, shifts could be scheduled in locations such as Cherokee Village, Horseshoe Bend, and Cave City.

Schedule:

  • Flexible hours; choose shifts between Monday and Sunday
  • Set your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage members in their homes
  • Review and evaluate prior and current medical history, medical conditions, and medication usage
  • Deliver clear education on diagnoses and prescribed medications
  • Identify gaps in care and recommend solutions to optimize patient outcomes
  • No treatment planning or prescription responsibilities required

Qualifications:

  • Valid Nurse Practitioner license in Arkansas
  • Board Certification (AANP or ANCC)
  • MSN from an accredited nursing school
  • Current BLS/CPR certification
  • At least 6 months of recent Nurse Practitioner experience is preferred
