Job Description

Make a real difference in patient care by providing home health risk assessments as a Nurse Practitioner in the greater Wyoming County area. In this per diem opportunity, you'll collaborate directly with Medicare Advantage plan members, conducting comprehensive assessments in the comfort of their homes. Schedule:

  • Flexible shifts available Monday through Sunday
  • Option to create your own work schedule
  • Minimum weekly or bi-weekly hourly commitment required

Core Responsibilities:

  • Perform annual and post-discharge home health risk assessments for Medicare Advantage members in Attica, Warsaw, Perry, and other Wyoming County locations
  • Review and evaluate each patient's medical and medication history
  • Educate patients about chronic conditions, medication usage, and recommended health practices
  • Identify potential gaps in care and recommend next stepsno requirement to prescribe medications or create treatment plans
  • Provide thorough documentation for each 45-minute assessment

Qualifications:

  • Active Nurse Practitioner license in New York
  • Board certified through AANP or ANCC
  • Current BLS certification (CPR)
  • Master of Science in Nursing (MSN) from an accredited program
  • At least 6 months of Nurse Practitioner experience is highly preferred
