Job Description
Advance your clinical expertise as a Nurse Practitioner conducting comprehensive home health risk assessments for Medicare Advantage members in Wyoming County, NY. This per diem opportunity offers both independence and flexibilitydesign your weekly or bi-weekly schedule, with potential assignments throughout Attica, Warsaw, Perry, and surrounding areas.
Schedule:
- Choose your own shifts, with availability between Monday and Sunday
- Flexible scheduling options to accommodate your commitments
- Minimum hourly commitment required weekly or bi-weekly
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments in members' residences
- Review and evaluate members current and prior medical histories, conditions, and medication profiles
- Educate patients regarding their health conditions and appropriate medication usage
- Identify gaps in care, recommending steps for improved outcomes
- No prescribing or creation of treatment plans involvedfocus is on assessment and care coordination
Qualifications:
- Active Nurse Practitioner license in NY State
- Board Certification through AANP or ANCC
- Current Basic Life Support (BLS/CPR) certification
- MSN degree from an accredited nursing program
- At least 6 months recent experience as a Nurse Practitioner (preferred)
