An exciting opportunity is available for a skilled Nurse Practitioner with home health experience to conduct risk assessments throughout Franklin County, VA, including Rocky Mount, Boones Mill, and Ferrum. This per diem role allows you to make a significant impact by providing personalized, in-home health evaluations for Medicare Advantage plan members. Enjoy the flexibility to shape your work schedule and engage with members within their homes, helping to ensure their well-being and close gaps in care.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own hours
- Commit to a minimum number of hours weekly or bi-weekly
Key Responsibilities:
- Perform comprehensive annual home health risk assessments in patient homes
- Review and evaluate past and current medical history, conditions, and medication usage
- Offer education to members regarding their health status and medication management
- Identify and communicate gaps in care with recommendations for next steps
- Focus strictly on assessments; no prescribing or creation of treatment plans required
Qualifications:
- Active Nurse Practitioner license in the state of Virginia
- CPR certification (BLS)
- Board Certification through AANP or ANCC required
- At least 6 months of current experience as a Nurse Practitioner preferred
- MSN degree from an accredited nursing program
Compensation: Earn $80-$90 per evaluation, plus a mileage stipend and administrative time.
