Embrace a rewarding per diem opportunity as a Nurse Practitioner specializing in home health risk assessments, serving Medicare Advantage members across Franklin County. This role offers the flexibility to build your own schedule with shifts available any day between Monday and Sunday, providing the autonomy to meet your lifestyle and professional goals. Assignments may include travel to areas such as Rocky Mount, Boones Mill, and Ferrum, where youll conduct in-home visits delivering thorough, patient-centered care.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual risk assessments for Medicare Advantage health plan members in their homes
- Assess prior and current medical history, including review of medical conditions and medications
- Deliver education to patients on their health conditions and treatment regimens
- Identify gaps in care, offering actionable recommendations to promote health and well-being
- No requirement to develop treatment plans or prescribe mediations
Qualifications:
- Active Nurse Practitioner license in Virginia
- Current CPR certification (BLS)
- Board certification via AANP or ANCC
- At least six months of recent experience as an NP preferred
- Master of Science in Nursing (MSN) from an accredited program
