Job Description

Help deliver vital healthcare services to the community with a rewarding per diem opportunity conducting home health risk assessments. This position offers Nurse Practitioners the chance to engage with Medicare Advantage members in their homes throughout Bingham County, providing meaningful interactions and clinical evaluations without the need for ongoing care management or prescription responsibilities.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

  • Perform comprehensive annual risk assessments in patient homes for a Medicare Advantage health plan
  • Review and evaluate current and past medical history, existing conditions, and medication regimens
  • Offer education on health conditions and prescribed medications to patients
  • Identify care gaps and recommend tailored steps for closure
  • Focus solely on assessmentsno prescribing medications or developing treatment plans required

Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Current Basic Life Support (BLS) CPR certification
  • Board certification from either AANP or ANCC
  • At least 6 months of recent Nurse Practitioner experience preferred
  • MSN degree from an accredited nursing school

This per evaluation role comes with a highly competitive earnings opportunity of $80-$90 per evaluation, plus a mileage stipend and compensation for administrative time.
