Skilled Nurse Practitioners with experience in home health are needed for impactful per diem opportunities conducting comprehensive in-home risk assessments. This role focuses on evaluating the health status of Medicare Advantage plan members throughout Barnwell County, SCincluding communities such as Barnwell, Williston, and Blackville. Assessments last approximately 45 minutes and may be annual wellness visits or post-discharge evaluations, offering vital insights without requiring the development of treatment plans or the prescription of medications.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule based on availability
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Complete in-home annual health risk assessments for Medicare Advantage members
• Review and evaluate prior and current medical history, ongoing conditions, and medication usage
• Offer education to patients regarding their conditions and prescribed medications
• Identify gaps in care and provide clear recommendations to address them
• Focus on evaluation and educationno prescribing or treatment plan creation is necessary

Qualifications:

• Active Nurse Practitioner license in South Carolina
• Current CPR (BLS) certification
• Board certified by AANP or ANCC
• MSN from an accredited nursing program
• Minimum of 6 months current Nurse Practitioner experience preferred
