Nurse Practitioners with a passion for home-based healthcare are invited to provide essential risk assessments to Medicare Advantage members throughout Stark County, Ohio. This per diem opportunity offers a flexible schedule and the autonomy to select your own shifts, ensuring an excellent work-life balance while making a significant impact in the community.
Schedule:
- Flexible shifts available between Monday and Sunday
- Choose your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete comprehensive home health risk assessments, each lasting approximately 45 minutes, for Medicare Advantage plan members in various areas within Stark County
- Assess prior and current medical history, ongoing medical conditions, and medication use
- Deliver patient-focused education on medical conditions and prescribed medications
- Recognize and document gaps in care, providing recommendations for further management
- Annual health assessments and post-discharge visits as assigned
- This position does not require the development of treatment plans or the prescribing of medications
Qualifications:
- Active Nurse Practitioner licensure in Ohio
- Current CPR certification (BLS)
- Board Certification (AANP or ANCC)
- MSN degree from an accredited nursing program
- At least 6 months of current NP experience is preferred
This role offers compensation of $80$90 per evaluation, in addition to a mileage stipend and pay for administrative time. Work within communities such as Canton, Massillon, North Canton, and Alliance, helping to enhance patient outcomes from the comfort of their own homes.
