Schedule:

Key Responsibilities:
- Conduct thorough home health risk assessments, typically lasting about 45 minutes, for Medicare Advantage plan members
- Complete annual health reviews and post-discharge assessments as scheduled
- Review medical history, current conditions, and medication regimens
- Provide clear, supportive education on conditions and medications during visits
- Identify care gaps and offer recommendations to resolve them
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board Certification via AANP or ANCC
- Minimum of 6 months recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
This position offers a competitive per-evaluation stipend between $80$90, along with mileage reimbursement and compensation for administrative time.
