Job Description
Make a meaningful impact on patient well-being by performing comprehensive home health risk assessments. As a Nurse Practitioner specializing in home-based care, you will serve Medicare Advantage plan members throughout Avery County, NC, including but not limited to Banner Elk, Beech Mountain, Crossnore, and Elk Park. This per diem, flexible role is designed for a motivated clinician seeking autonomy in creating their own schedule.
Schedule:
- Flexible shifts offered every day, Monday through Sunday
- Option to create your own schedule
- Minimum weekly or bi-weekly hourly commitment based on preference
Key Responsibilities:
- Conduct annual or post-discharge home health risk assessments (approximately 45 minutes per evaluation)
- Assess and document patients prior and current medical history, ongoing health conditions, and complete medication reviews
- Deliver patient education regarding chronic conditions, preventive care, and medication usage
- Identify gaps in care and provide actionable recommendations for resolution
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR (BLS) certification
- Board Certification from AANP or ANCC
- At least six months of recent experience as a Nurse Practitioner is preferred
- Master of Science in Nursing (MSN) from an accredited institution
