This per diem Nurse Practitioner role is tailored for professionals seeking flexibility, with opportunities to conduct home health risk assessments for a Medicare Advantage health plan across Barnwell County. Your visitseach approximately 45 minutesmay involve annual health evaluations or post-discharge follow-ups, ensuring members receive attentive, one-on-one care without the demands of ongoing treatment or prescribing.

• Flexible shifts available Monday through Sunday

• Commit to a minimum number of hours each week or every other week

Key Responsibilities:

• Conduct thorough annual home health risk assessments for Medicare Advantage plan members
• Review prior and current medical history, medical conditions, and medication lists
• Educate patients about their diagnoses and medications, supporting self-management
• Detect care gaps and make actionable recommendations to address them
• Assessments only; no ongoing patient management or prescription writing required

Qualifications:

• Active South Carolina Nurse Practitioner license
• Board Certification (AANP or ANCC)
• Current Basic Life Support (CPR/BLS) certification
• MSN degree from an accredited school of nursing
• At least 6 months of recent NP experience preferred
