Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage plan members
• Review patient medical history, evaluate medical conditions, and assess medication use
• Provide patient education regarding diagnoses and medication management
• Detect care gaps and offer actionable recommendations to promote patient wellness
• No responsibility for prescribing medication or developing treatment plans

Preferred Qualifications:

• Current Nurse Practitioner license in Missouri
• Board Certification through AANP or ANCC
• Valid CPR certification (BLS)
• MSN degree from an accredited nursing program
• Minimum 6 months of recent nurse practitioner experience preferred
