Job Description

Embark on a flexible, impactful role conducting home health risk assessments within Saratoga County. As a nurse practitioner specializing in home health, you will visit Medicare Advantage plan members, providing essential annual health assessments or post-discharge visits. Each risk assessment typically takes about 45 minutes and may be scheduled in multiple areas throughout the county, including Clifton Park, Saratoga Springs, and Halfmoon.

Schedule:

  • Flexible shifts available seven days a week
  • Self-scheduled; create shifts that fit your life
  • Weekly or bi-weekly minimum hour commitments

Key Responsibilities:

  • Carry out comprehensive home health risk assessments for Medicare Advantage members
  • Review and document patients medical histories, conditions, and medication usage
  • Educate patients about their medical conditions and proper medication management
  • Identify care gaps, addressing them with actionable recommendations
  • No requirements for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in New York
  • Board certification through AANP or ANCC
  • Current BLS/CPR certification
  • Minimum of six months experience as an NP (preferred)
  • Master of Science in Nursing (MSN) from an accredited program

This per diem engagement offers an attractive rate of $80-$90 per completed evaluation, including a mileage stipend and compensation for administrative time, all while allowing you to manage your own schedule.
