Job Description
Deliver compassionate, patient-centered care as a Nurse Practitioner specializing in home health risk assessments throughout Saratoga County, NY. This per diem role offers the flexibility to create your own schedule, providing opportunities to work shifts of your choice between Monday and Sunday, with a minimum weekly or bi-weekly hourly commitment.

Key Responsibilities:
- Conduct thorough annual health risk assessments for Medicare Advantage plan members in Saratoga County, including Saratoga Springs, Clifton Park, and Halfmoon.
- Evaluate prior and current medical history, medical conditions, and comprehensive medication history during in-home visits, each approximately 45 minutes.
- Educate patients on their medical conditions and medications, ensuring clear understanding and ongoing engagement in their health.
- Identify and document any gaps in care, providing targeted recommendations for resolution.
- No requirement to create treatment plans or prescribe medications.
Qualifications:
- Active Nurse Practitioner License in New York State
- Current CPR certification (BLS)
- Board Certification via AANP or ANCC
- MSN degree from an accredited nursing program
- Minimum six months of recent NP experience preferred
This position offers competitive per evaluation compensation, with each completed assessment compensated at $80-$90, plus an additional mileage stipend and payment for administrative time.
