Job Description

Experienced Nurse Practitioners are sought for per diem assignments conducting home health risk assessments in Schenectady County, NY. In this role, you will support Medicare Advantage members by performing comprehensive, in-home health evaluations. Each visit typically takes around 45 minutes and could be an annual assessment or a post-discharge review. With a flexible schedule and the ability to set your own hours, this position is ideal for those seeking flexibility and autonomy in their practice. Candidates must be open to traveling to various locations within Schenectady County, such as Schenectady, Rotterdam, and Glenville.

Schedule

  • Flexible shifts available between Monday and Sunday
  • Control your own schedule with a minimum weekly or bi-weekly hourly commitment

Responsibilities

  • Complete annual home health risk assessments for Medicare Advantage members
  • Review and evaluate prior and current medical history, medical conditions, and medications
  • Educate patients about their health conditions and medications
  • Identify care gaps and recommend steps for resolution
  • No responsibility for prescribing medications or developing treatment plans

Qualifications

  • Active Nurse Practitioner license in New York State
  • Current Basic Life Support (CPR) certification
  • Board certification from AANP or ANCC
  • Minimum of 6 months' recent NP experience preferred
  • Master's degree (MSN) from an accredited nursing program
