Job Description
Advance your career in home health by performing comprehensive risk assessments for Medicare Advantage members throughout Onondaga County. As a Nurse Practitioner with expertise in home health, you will visit individuals in their homesincluding areas such as Salina, Onondaga, Clay, and Syracuseto evaluate overall health, provide education, and support better healthcare outcomes.
Schedule:
- Flexible shifts available between Monday and Sunday
- Ability to create your own schedule
- Set your minimum weekly or bi-weekly hour commitment
Key Responsibilities:
- Conduct detailed home health risk assessments, averaging 45 minutes each
- Assess prior and current medical history, conditions, and medication use
- Deliver clear education regarding medical conditions and medications
- Detect gaps in care, offering recommendations to address them
- No responsibilities for creating treatment plans or prescribing medications
Qualifications:
- Current Nurse Practitioner license in New York
- BLS certification (CPR)
- Board certification through AANP or ANCC
- MSN from an accredited nursing school
- At least 6 months' recent experience as a Nurse Practitioner preferred
This per diem opportunity provides an hourly evaluation rate of $80$90, with additional mileage and administrative stipends.
