Deliver high-quality patient care by conducting home health risk assessments as a Nurse Practitioner serving Medicare Advantage plan members throughout Onondaga County, NY. This per diem opportunity allows for exceptional flexibility, making it ideal for dedicated clinicians seeking to shape their own schedules while making a meaningful community impact. Shifts are available any day of the week, including weekends, with the ability to choose your preferred hours and a manageable minimum hourly commitment per week or bi-weekly period.
Schedule:
- Flexible scheduling between Monday and Sunday
- Create your own shift schedule to fit your needs
- Minimum required hours per week or every two weeks
Key Responsibilities:
- Complete comprehensive annual home health risk assessments, typically lasting around 45 minutes, at patients' residences
- Review and evaluate patients prior and current medical history, ongoing medical conditions, and medication usage
- Educate patients on specific medical conditions and prescribed medications to promote understanding and healthy living
- Identify care gaps and provide effective recommendations to address them
- Focus solely on assessmentno requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in NY State
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months recent experience working as a Nurse Practitioner (preferred)
- MSN degree from an accredited nursing program
