Key Responsibilities
- Perform comprehensive annual home health risk assessments for Medicare Advantage members residing in various areas within White County
- Assess prior and current medical histories, ongoing conditions, and medication regimens
- Deliver patient education regarding medical conditions and medications
- Identify gaps in care and make informed recommendations to improve overall health outcomes
- Focus purely on assessmentsno requirement for developing treatment plans or prescribing medications
Qualifications
- Active Nurse Practitioner license in Arkansas
- Board Certified through AANP or ANCC
- Current CPR (BLS) certification
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of current Nurse Practitioner experience is preferred
