This per diem opportunity offers the flexibility to design your own schedule, with available shifts spanning Monday through Sunday. Schedule:

• Flexible shifts available MondaySunday
• Design your own work schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive home health risk assessments, averaging 45 minutes each, for Medicare Advantage members
• Evaluate patients prior/current medical histories, chronic conditions, and medications
• Educate patients about their health conditions and prescribed treatments
• Identify care gaps, document findings, and provide recommendations
• No requirement for treatment plan creation or medication prescribing

Desired Qualifications:

• Active Nurse Practitioner license in North Carolina
• Current CPR (BLS) certification
• Board Certified through AANP or ANCC
• MSN from an accredited institution
• Minimum six months of recent Nurse Practitioner experience preferred
