Deliver compassionate and impactful care as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity offers the flexibility to set your own schedule, with shifts available any day of the week. Youll support Medicare Advantage members throughout various communities in Beaufort County, providing valuable assessments in patient homes.
Schedule:
- Flexible shifts Monday through Sunday
- Self-schedulingdesign shifts to fit your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual health risk assessments for Medicare Advantage members in their homes
- Gather and evaluate prior and current medical history, health conditions, and medication profiles
- Provide education regarding diagnoses, medications, and preventative care
- Identify care gaps and offer actionable recommendations to improve patient outcomes
- Visits do not require treatment planning or prescribing
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current BLS/CPR certification
- Board-certified by AANP or ANCC
- MSN from an accredited nursing program
- Minimum of six months of experience as a Nurse Practitioner preferred
