Take the lead in supporting patient health outcomes as a Nurse Practitioner conducting home health risk assessments in Pitt County, NC. This per diem role offers flexibility and autonomy, with visits primarily to Medicare Advantage plan members in locations such as Greenville, Winterville, and Ayden. Each assessment, lasting about 45 minutes, is an opportunity to make a meaningful impact through comprehensive evaluation and patient education.
Schedule:
- Flexible shifts available between Monday and Sunday
- Self-scheduling: create your own hours
- Choose between weekly or bi-weekly minimum hourly commitments
Key Responsibilities:
- Perform thorough annual or post-discharge risk assessments for Medicare Advantage health plan members in their homes
- Review, document, and evaluate patients medical history, current conditions, and medication regimens
- Provide clear education to patients regarding their health conditions and medications
- Identify care gaps and recommend actionable steps to improve health outcomes
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- Minimum of 6 months recent experience as a Nurse Practitioner strongly preferred
This opportunity comes with compensation of $80-$90 per evaluation, as well as a mileage stipend and administrative time.
