As a Nurse Practitioner, you will perform risk assessments for Medicare Advantage plan members in various locations within Hendricks County, such as Plainfield, Brownsburg, and Avon, IN. Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit personal and professional needs
- Minimum weekly or bi-weekly hourly commitments to suit your availability
Responsibilities:
- Perform comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members
- Assess current and past medical histories, chronic medical conditions, and medication use
- Educate patients on medical conditions and medication management
- Identify care gaps and make recommendations to close them
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Indiana
- Master's degree in Nursing (MSN) from an accredited institution
- Current CPR (BLS) certification
- Board certification via AANP or ANCC
- Minimum 6 months recent NP experience preferred
Nurse Practitioners in this per diem role enjoy compensation of $80$90 per evaluation, with additional mileage and administrative stipends included.
