This role focuses on supporting Medicare Advantage plan members by conducting thorough in-home assessments and providing educationmaking a direct difference in community care.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Create your own work schedule
• Minimum weekly or bi-weekly hour commitment

Key Responsibilities:

• Perform comprehensive, annual home health risk assessments for Medicare Advantage members
• Evaluate prior and current medical and medication histories
• Educate patients on conditions and prescribed medications, empowering informed health management
• Identify care gaps and offer actionable recommendations to resolve them
• There is no need to generate treatment plans or prescribe medications in this role

Qualifications:

• Active Nurse Practitioner license in Indiana
• Board Certification via AANP or ANCC
• Current CPR certification (BLS)
• Minimum MSN from an accredited school of nursing
• At least 6 months of recent nurse practitioner experience is preferred

This per diem opportunity provides a compensation structure of $80$90 per completed evaluation, along with a mileage stipend and compensation for administrative time. The work is conducted in a variety of locations within Hendricks County, such as Plainfield, Brownsburg, and Avon, allowing exposure to a diverse patient population and varied clinical settings.
