This per diem opportunity allows you to provide essential care to Medicare Advantage plan members residing in Mexico, Vandalia, Laddonia, and nearby areas. Schedule:
- Flexible shifts throughout the week, Monday to Sunday
- Self-scheduling: tailor your shifts to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment for optimal balance
Key Responsibilities:
- Perform comprehensive home health risk assessments with an average visit time of 45 minutes
- Review and evaluate prior and current medical history, conditions, and medications
- Educate patients on their health conditions and medication regimen
- Identify gaps in care and offer recommendations that promote patient wellbeing
- Visits may be annual health assessments or post-discharge evaluations
- Note: No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Missouri (MO)
- Board Certification via AANP or ANCC required
- Current BLS (CPR) certification
- Master of Science in Nursing (MSN) from an accredited institution
- Six months or more of recent NP experience highly preferred
