Deliver expert care in patients homes as a per diem Nurse Practitioner specializing in home health risk assessments. This opportunity allows you to provide invaluable support to Medicare Advantage members throughout Delta County, including Delta, Orchard City, and Paonia. Enjoy the autonomy and flexibility to craft your own work schedule while making a direct impact on patient outcomes.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Choose your own hours to create a personalized work schedule
- Meet a minimal commitment, scheduled weekly or bi-weekly
Key Responsibilities:
- Perform in-home annual risk assessments for Medicare Advantage plan members
- Review prior and current medical histories, medical conditions, and medication lists
- Educate patients and families on health conditions and medication regimens
- Detect gaps in care and recommend practical solutions
- No duties related to prescribing medications or creating treatment plans
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current BLS (CPR) certification
- Board certification through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months experience as a practicing NP preferred
This per diem position offers $80-$90 per completed evaluation, along with a mileage stipend and compensation for administrative time. Youll have the freedom to balance your career with lifes demands while serving your local community.
