Job Description

Seeking an experienced Nurse Practitioner with a passion for home health to perform risk assessments for Medicare Advantage members. This per diem opportunity offers flexible scheduling and the ability to create your own hours, accommodating those looking for autonomy in their professional routine.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Ability to build your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage health plan participants
  • Review and evaluate both current and prior medical history, chronic conditions, and medication usage
  • Educate patients on health conditions and proper use of medications
  • Identify care gaps and provide recommendations for follow-up, without the need to write treatment plans or prescribe
  • Visits typically last around 45 minutes and may involve annual assessments or post-discharge check-ins
  • Serve patients residing throughout Dearborn County, including areas such as Lawrenceburg, Greendale, and Aurora; scheduling may include travel within these locations

Qualifications:

  • Current Nurse Practitioner license in Indiana (IN)
  • Active CPR certification (BLS required)
  • Board Certification through AANP or ANCC
  • Minimum of 6 months of recent Nurse Practitioner experience preferred
  • MSN degree earned from an accredited nursing program

This role offers a rate of $80$90 per completed evaluation, with an additional mileage stipend and compensation for administrative time, providing strong earning potential for motivated professionals.
