This position is ideal for Nurse Practitioners who enjoy autonomy, patient-focused care, and setting their own schedule while serving Medicare Advantage members throughout Jefferson County, Colorado, including areas such as Lakewood, Arvada, and Westminster.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members in their homes
- Review and evaluate patients' past and current medical history, existing medical conditions, and medication regimens
- Offer education to patients about medical conditions and medication usage
- Identify and document any gaps in care, making recommendations for follow-up or resolution
- No creation of treatment plans or prescription writing involved
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current CPR certification (BLS)
- Board-certified through AANP or ANCC
- Minimum of 6 months current experience as a Nurse Practitioner preferred
- MSN degree earned from an accredited nursing school
