Schedule:
- Flexible shifts, available Monday through Sunday
- Design your own schedule to fit your lifestyle
- Commitment can be set on a weekly or bi-weekly basis
Key Responsibilities:
- Complete annual and post-discharge home health risk assessments, approximately 45 minutes per visit
- Assess and document comprehensive prior/current medical history, existing conditions, and medication usage for each patient
- Educate patients about their medical conditions and medication regimens to foster informed health choices
- Detect any gaps in care, offering actionable recommendations to support improved patient outcomes
- Focus exclusively on assessmentsno treatment planning or prescribing required
Desired Qualifications:
- Current Nurse Practitioner (NP) license in Colorado
- Active BLS/CPR certification
- Board Certification via AANP or ANCC
- A minimum of 6 months' experience in a Nurse Practitioner role preferred
- MSN degree from an accredited school of nursing
Compensation for this impactful work includes $80-$90 per completed assessment, as well as mileage and administrative time stipends, offering both rewarding work and competitive pay.
