Job Description

Take on a vital role as a Nurse Practitioner specializing in home health, conducting risk assessments for Medicare Advantage plan members in the Aurora, Thornton, and Westminster areas of Adams County, CO. This per diem opportunity offers you the autonomy to shape your schedule, working flexible shifts any day of the week, and determine your own workload with minimum weekly or bi-weekly commitments.

Schedule:

  • Flexible shifts between Monday and Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Primary Responsibilities:

  • Perform comprehensive home health risk assessments (approx. 45 minutes per visit)
  • Review and evaluate patients prior and current medical history, medical conditions, and medication use
  • Educate patients about their medical conditions and medications
  • Identify care gaps and recommend appropriate steps for resolution
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Colorado
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited nursing program
  • At least 6 months of recent NP experience preferred

In this position, youll be compensated $80$90 per evaluation, and provided with a mileage stipend and administrative time, reflecting the importance and flexibility of your work.
