Job Description

Shape meaningful patient outcomes in a dynamic role providing home health risk assessments for Medicare Advantage plan members. This per diem Nurse Practitioner position offers the flexibility to craft your own schedule, with opportunities to work throughout locations in Hertford County as part of a dedicated home health initiative. You'll deliver comprehensive and compassionate care in patients homeswhether performing annual assessments or post-discharge visitshelping bridge gaps in care and supporting healthy aging at home.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule for optimal work-life balance
  • Commit to a minimum number of hours weekly or bi-weekly to suit your needs

Key Responsibilities:

  • Conduct thorough home-based risk assessments for Medicare Advantage members
  • Review medical history, existing conditions, and current medication regimens
  • Offer patient-centered education on health conditions and medications
  • Identify and document gaps in care with actionable recommendations
  • No responsibility for treatment planning or medication prescribing

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current CPR certification (BLS)
  • Board certification as an NP through AANP or ANCC
  • MSN degree from an accredited nursing program
  • At least six months NP experience preferred, particularly in home health
