This per diem position allows for maximum flexibility as you support individuals across multiple communities, ensuring they receive quality preventive care and support in their own homes.
Schedule:
- Flexible shifts available Monday through Sunday

- Commitment can be set on a weekly or bi-weekly basis
Key Responsibilities:
- Conduct comprehensive annual or post-discharge home health risk assessments for Medicare Advantage members
- Review and evaluate clients medical history, current conditions, and medication regimen
- Educate clients on their health conditions and medications, ensuring understanding and promoting health literacy
- Identify gaps in care and recommend solutions to improve patient outcomes
- Perform visit documentation without the need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board certified via AANP or ANCC
- MSN degree from an accredited nursing school
- At least 6 months of NP experience preferred
This per diem role offers competitive compensation at $80$90 per completed evaluation. In addition, a mileage stipend and administrative time compensation are included, recognizing the demands and value of your time spent supporting local communities.
