Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum hourly commitment required on a weekly or bi-weekly basis
Responsibilities:
- Conduct comprehensive health risk assessments for Medicare Advantage members in Augusta, McCrory, Cotton Plant, and throughout Woodruff County, AR
- Assess and document prior/current medical history, conditions, and medications during 45-minute home visits
- Educate patients on their medical conditions and prescribed medications
- Identify care gaps, providing actionable recommendations to improve patient outcomes
- Focus exclusively on assessment and patient educationno prescribing or treatment planning required
Qualifications:
- Active Nurse Practitioner license in Arkansas
- CPR certification (BLS) required
- Board certification through AANP or ANCC
- Masters degree (MSN) from an accredited nursing program
- Minimum six months of nurse practitioner experience preferred
