Job Description
Seeking a dedicated Nurse Practitioner with experience in home health to conduct risk assessments for Medicare Advantage plan members throughout Cayuga County. This per diem opportunity offers extraordinary flexibility and autonomy, perfect for NPs who want to create their own schedules while making a meaningful impact in the community.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule to best suit your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments in patient homes; each visit averages 45 minutes
- Evaluate prior and current medical histories, medical conditions, and medication histories
- Provide education to patients regarding their conditions and medications
- Identify gaps in care and share recommendations for follow-up with the care team
- Visits may include annual assessments or post-discharge evaluations
- No responsibility for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in NY
- Current CPR (BLS) certification
- Board certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Minimum 6 months recent NP experience preferred (home health experience a plus)
This position offers a highly competitive per-evaluation rate between $80$90. Additionally, a mileage stipend and compensation for administrative time are provided to ensure your travel and documentation efforts are recognized and supported.
