Job Description

Advance your clinical skills in a dynamic, patient-focused role as a Nurse Practitioner serving home health clients in Cayuga County, NY. As a key member of the care team, you will perform comprehensive home health risk assessments for Medicare Advantage plan members residing throughout Auburn and surrounding areas, including Cayuga, Union Springs, and Moravia. This opportunity offers per diem flexibility, allowing you to create a schedule that suits your lifestylechoose from shifts available any day of the week and set your own minimum weekly or bi-weekly commitment.

Schedule:

  • Flexible shifts between Monday and Sunday
  • Self-scheduled shifts
  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Conduct timely annual home health risk assessments during home visits (typically 45 minutes each)
  • Review and evaluate current and prior medical history, conditions, and medications
  • Provide thoughtful education about medical conditions and medication regimens to patients
  • Recognize care gaps and offer actionable recommendations to ensure optimal patient outcomes
  • No treatment plan creation or medication prescribing required

Qualifications:

  • Active Nurse Practitioner License in NY
  • Current CPR certification (BLS)
  • Board Certification via AANP or ANCC
  • Completion of MSN from an accredited nursing program
  • At least 6 months of experience as a Nurse Practitioner preferred

In this per diem position, youll receive $80-$90 for each evaluationplus a mileage stipend and administrative time compensation. This allows you to maximize your earnings while delivering essential care and education to patients in a supportive environment.
