Job Description

Take on a dynamic per diem opportunity providing impactful home health services across Shawnee County. As a Nurse Practitioner specializing in home health, you will conduct comprehensive health risk assessments for Medicare Advantage plan members in areas such as Topeka, Silver Lake, and Auburn.

Schedule:

  • Flexible shifts available seven days a week
  • Self-select your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform in-home risk assessments, averaging 45 minutes each, for Medicare Advantage members
  • Review and evaluate prior/current medical history, chronic conditions, and medication regimens
  • Offer patient education regarding health conditions and prescribed medications
  • Identify gaps in patient care, providing recommendations for improvement
  • Note: this position does not require creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Kansas
  • Board Certification through AANP or ANCC
  • Current BLS (CPR) certification
  • MSN from an accredited nursing program
  • At least six months of recent experience as a Nurse Practitioner is preferred
