Job Description
Take on a dynamic per diem opportunity providing impactful home health services across Shawnee County. As a Nurse Practitioner specializing in home health, you will conduct comprehensive health risk assessments for Medicare Advantage plan members in areas such as Topeka, Silver Lake, and Auburn.
Schedule:
- Flexible shifts available seven days a week
- Self-select your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform in-home risk assessments, averaging 45 minutes each, for Medicare Advantage members
- Review and evaluate prior/current medical history, chronic conditions, and medication regimens
- Offer patient education regarding health conditions and prescribed medications
- Identify gaps in patient care, providing recommendations for improvement
- Note: this position does not require creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Kansas
- Board Certification through AANP or ANCC
- Current BLS (CPR) certification
- MSN from an accredited nursing program
- At least six months of recent experience as a Nurse Practitioner is preferred
