Advance your career as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity empowers you to shape your own schedule and serve Medicare Advantage plan members in Attica and the greater Wyoming County, NY regionincluding Warsaw and Perry. Visits typically last around 45 minutes and may include annual health assessments or post-discharge follow-ups, ensuring meaningful engagements with patients where they are most comfortable.

Key Responsibilities:
- Conduct thorough, in-home health risk assessments for Medicare Advantage members
- Review patients prior and current medical history, conditions, and medication usage
- Educate individuals on their health conditions and prescribed medications to promote understanding and self-management
- Identify gaps in care and offer actionable recommendations for care coordination and improvement
- No requirement to develop treatment plans or prescribe medication
Qualifications:
- Valid Nurse Practitioner license in NY
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current CPR certification (BLS)
- Six months or more of recent Nurse Practitioner experience is preferred
