Deliver essential care in the community by performing home health risk assessments for Medicare Advantage plan members throughout Fountain County, IN. This per diem opportunity empowers nurse practitioners to leverage flexible scheduling optionswork as your calendar allows, with shifts available across Attica, Covington, and Veedersburg. Schedule:

• Create your own schedule
• Flexible shifts, Monday through Sunday
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct annual home health risk assessments for Medicare Advantage members
• Review prior and current medical history, existing conditions, and medication lists
• Educate patients on diagnoses and medication use
• Identify care gaps and recommend steps to address them
• Visits average about 45 minutes and may include post-discharge checks or annual assessments
• No treatment plan development or medication prescribing necessary

Qualifications:

• Active Nurse Practitioner license in Indiana
• Current CPR certification (BLS)
• Board certification through AANP or ANCC required
• MSN from an accredited nursing school
• Minimum 6 months recent experience as a nurse practitioner preferred
