Embark on a rewarding per diem opportunity as a Nurse Practitioner specializing in home health risk assessments within Fountain County, Indiana, including Attica, Covington, and Veedersburg. In this role, you will perform thorough, patient-centered home visits for Medicare Advantage plan members, focusing on preventive care and gap identification.
Schedule:
- Flexible shifts available any day of the week (Monday-Sunday)
- Self-scheduling to accommodate your commitments
- Set your own minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual risk assessments and post-discharge visits in patients homes (each visit averages 45 minutes)
- Assess prior and current medical histories, ongoing health conditions, and medication regimens
- Deliver education to patients on disease management and medication adherence
- Identify care gaps, making actionable recommendations for improved outcomes
- No requirement to create treatment plans or prescribe medicationsfocus purely on assessment and education
Qualifications:
- Current Nurse Practitioner license in Indiana
- Certification in Basic Life Support (CPR/BLS)
- National board certification (AANP or ANCC)
- Minimum of six months recent nurse practitioner experience preferred
- MSN degree from an accredited school of nursing
