Job Description

Embark on a rewarding per diem opportunity as a Nurse Practitioner specializing in home health risk assessments within Fountain County, Indiana, including Attica, Covington, and Veedersburg. In this role, you will perform thorough, patient-centered home visits for Medicare Advantage plan members, focusing on preventive care and gap identification.

Schedule:

  • Flexible shifts available any day of the week (Monday-Sunday)
  • Self-scheduling to accommodate your commitments
  • Set your own minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive annual risk assessments and post-discharge visits in patients homes (each visit averages 45 minutes)
  • Assess prior and current medical histories, ongoing health conditions, and medication regimens
  • Deliver education to patients on disease management and medication adherence
  • Identify care gaps, making actionable recommendations for improved outcomes
  • No requirement to create treatment plans or prescribe medicationsfocus purely on assessment and education

Qualifications:

  • Current Nurse Practitioner license in Indiana
  • Certification in Basic Life Support (CPR/BLS)
  • National board certification (AANP or ANCC)
  • Minimum of six months recent nurse practitioner experience preferred
  • MSN degree from an accredited school of nursing
