Elevate your nursing career with a per diem opportunity focused on conducting home health risk assessments for Medicare Advantage plan members in Douglas County, GA. As a Nurse Practitioner in this role, youll engage with patients in their homes across Douglasville, Villa Rica, and Austell, providing critical assessments that help shape patient wellness and care coordination.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive annual health risk assessments for Medicare Advantage members in the community
• Review and evaluate patients prior and current medical histories, conditions, and medication usage
• Deliver patient education regarding medical conditions and medication regimens
• Identify gaps in care and offer recommendations to ensure comprehensive coverage and improved outcomes
• No direct prescribing of medications or development of treatment plans required

Qualifications:

• Active Nurse Practitioner License in Georgia
• Board Certification through AANP or ANCC
• Current BLS (CPR) certification
• Minimum of 6 months current Nurse Practitioner experience (preferred)
• Master of Science in Nursing (MSN) from an accredited institution
