This per diem opportunity offers flexibility and the chance to make a meaningful impact on Medicare Advantage members throughout Pope County, AR, including Russellville, Pottsville, and Atkins. As a Nurse Practitioner, you will conduct thorough home health risk assessments, guiding patients to improved understanding and management of their medical conditionsall while maintaining autonomy over your work schedule.
Schedule:
- Flexible shifts available, Monday through Sunday
- Design your own schedule to fit your lifestyle
- Choose your minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Review current and prior medical history, including chronic conditions and medication use
- Educate patients on their medical diagnoses and prescribed medications
- Identify care gaps and recommend appropriate follow-up, without the responsibility of creating treatment plans or prescribing medications
- Accurately complete documentation for each 45-minute evaluation
Desired Qualifications:
- Active Nurse Practitioner license in Arkansas
- Board Certification via AANP or ANCC
- Current BLS/CPR certification
- MSN degree from an accredited nursing program
- At least six months of recent NP experience, particularly in home health (preferred)
