Advance your clinical expertise in a dynamic, patient-focused position as a Nurse Practitioner in home health. Provide essential risk assessments to Medicare Advantage plan members across Kootenai County, including communities such as Coeur d'Alene, Post Falls, and Hayden. This per diem opportunity is perfect for Nurse Practitioners seeking flexibility and meaningful direct patient impact.
Schedule
- Flexible shifts available all days of the week (MondaySunday)
- Design your own schedule based on availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities
- Conduct comprehensive annual and post-discharge home health risk assessments, averaging 45 minutes per visit
- Thoroughly review patients' medical history, current health conditions, and medication use
- Deliver patient education on medical conditions and prescribed medications
- Detect any gaps in care and communicate recommendations for follow-up
- No requirement for prescribing medications or developing new treatment plans
Qualifications
- Valid Nurse Practitioner license in Idaho
- MSN from an accredited nursing program
- Active Board Certification (AANP or ANCC)
- Current CPR (BLS) certification
- At least 6 months of experience as an NP preferred
