Embark on a meaningful per diem opportunity as a Nurse Practitioner delivering personalized care in a home health setting. This role focuses on home health risk assessments for Medicare Advantage members across various communities. Assignments may include Athens, Princeton, and Bluefield, accommodating shifts within Kanawha County, WV. Tailor your own work schedule with flexible shift selection, and choose a commitment that matches your availability.
Schedule:
- Flexible shifts offered Monday through Sunday
- Design your own weekly schedule
- Commit to a minimum number of hours, weekly or bi-weekly
Responsibilities:
- Perform comprehensive home health risk assessments, averaging 45 minutes per evaluation
- Assess prior and current medical histories, existing conditions, and medication regimens
- Educate patients on diagnosed conditions and medication use
- Detect gaps in care and suggest appropriate actions for improvement
- Visits may include annual health assessments or post-discharge check-ins
- No prescribing, no treatment plan developmentall focus is on evaluation and education
Qualifications:
- Licensed as a Nurse Practitioner in WV
- Active BLS (CPR) certification required
- Board certified by either the AANP or ANCC
- At least six months of Nurse Practitioner experience preferred
- MSN from an accredited nursing program
