Job Description

Deliver essential home healthcare services by conducting comprehensive risk assessments for Medicare Advantage plan members in McMinn County. As a Nurse Practitioner specializing in home health, you'll play a pivotal role in supporting patient wellness through thorough evaluations, education, and care recommendationswithout the pressure of ongoing treatment planning or medication prescribing.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule and choose shifts to fit your lifestyle
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct annual risk assessments in patients homes (typically 45 minutes each)
  • Assess medical history, current conditions, and medication use
  • Offer education on medical conditions and medications to patients
  • Identify gaps in care and make documented recommendations
  • Complete patient visits in Athens and surrounding McMinn County locations (Etowah, Englewood)
  • Support improved health outcomes through exceptional attention to detail and patient-centered care

Qualifications:

  • Licensed Nurse Practitioner in TN
  • Current BLS CPR certification
  • Board certified via AANP or ANCC
  • MSN from an accredited nursing program
  • At least 6 months recent Nurse Practitioner experience preferred
  • Strong interpersonal and communication skills
  • Able to travel to patient residences within McMinn County
