Experience a rewarding opportunity as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members throughout Westchester County, NY. This per diem position is ideal for clinicians who thrive in autonomous roles and value scheduling flexibilitycreate your own shifts and commit to a minimum number of hours each week or every two weeks. Assignments may occur throughout Westchester County, including New Rochelle, Mount Vernon, White Plains, and Yonkers.
Schedule:
- Flexible shifts available between Monday and Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments in members homes
- Review prior and current medical history, conditions, and medications
- Educate members on medical conditions and prescribed medications
- Identify care gaps and provide clear recommendations to close them
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in New York
- Current BLS CPR certification
- Board Certification through AANP or ANCC
- Minimum of 6 months recent NP experience preferred
- MSN from an accredited nursing program
