This per diem role offers a high level of flexibility with shifts available any day of the week and the ability to design your own schedule, making it ideal for clinicians seeking a dynamic work-life balance. Assignments span Yonkers, New Rochelle, Mount Vernon, White Plains, and neighboring areas, and each home health risk assessment typically requires about 45 minutes to complete.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own hours each week
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct thorough annual health risk assessments in members homes
- Review each patients prior and current medical, medication, and condition history
- Educate patients regarding their health, treatments, and medications
- Identify gaps in care and recommend next steps for optimal health outcomes
- Responsible for assessments onlyno prescribing or treatment planning required
Qualifications:
- Active Nurse Practitioner license in the state of New York
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months experience as an NP preferred
- Master of Science in Nursing (MSN) from an accredited institution
