Job Description
Deliver essential home-based healthcare as a Nurse Practitioner conducting risk assessments for Medicare Advantage plan members across Jackson County, OR. This per diem opportunity allows you to make a meaningful impact by supporting individuals in Ashland, Medford, Eagle Point, Jacksonville, and nearby communitiesall on a highly flexible schedule you control.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling: create your own work calendar
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members in their residences
- Review and evaluate medical history, current conditions, and medication lists
- Educate patients on health conditions and medications
- Identify gaps in care and provide recommendations to address potential health risks
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Valid Nurse Practitioner license in Oregon
- Current Basic Life Support (BLS) certification
- Board Certification as a Nurse Practitioner via AANP or ANCC
- Minimum of 6 months current NP experience preferred
- MSN degree from an accredited nursing program
You will receive compensation of $80 to $90 per evaluation, plus a mileage stipend and additional pay for administrative time, acknowledging your expertise and dedication to patient care.
