Take your Nurse Practitioner expertise into the community by providing in-home risk assessments for Medicare Advantage plan members in picturesque Madison County. In this per diem opportunity, youll play a crucial role in promoting preventive care while enjoying flexibility in your schedule.
Schedule:
- Flexible shifts available any day of the week (MondaySunday)

Primary Responsibilities:
- Conduct comprehensive home health risk assessments, typically lasting around 45 minutes per visit
- Perform annual health assessments or post-discharge visits as assigned
- Assess prior and current medical history, document ongoing medical conditions, and review medication history
- Provide patient education on diagnoses and prescribed medications
- Identify gaps in care and offer guidance to optimize patient outcomes
- All visits are strictly assessment-basedno prescribing or treatment planning required
Desired Qualifications:
- Valid Nurse Practitioner license in North Carolina
- Board certification through AANP or ANCC
- Current CPR certification (BLS)
- Graduation from an accredited MSN program
- Preference for candidates with at least 6 months of nurse practitioner experience
- A passion for patient-centered, proactive healthcare delivered in the home setting
