Job Description

Take your Nurse Practitioner expertise into the community by providing in-home risk assessments for Medicare Advantage plan members in picturesque Madison County. In this per diem opportunity, youll play a crucial role in promoting preventive care while enjoying flexibility in your schedule.

Schedule:

  • Flexible shifts available any day of the week (MondaySunday)

Primary Responsibilities:

  • Conduct comprehensive home health risk assessments, typically lasting around 45 minutes per visit
  • Perform annual health assessments or post-discharge visits as assigned
  • Assess prior and current medical history, document ongoing medical conditions, and review medication history
  • Provide patient education on diagnoses and prescribed medications
  • Identify gaps in care and offer guidance to optimize patient outcomes
  • All visits are strictly assessment-basedno prescribing or treatment planning required

Desired Qualifications:

  • Valid Nurse Practitioner license in North Carolina
  • Board certification through AANP or ANCC
  • Current CPR certification (BLS)
  • Graduation from an accredited MSN program
  • Preference for candidates with at least 6 months of nurse practitioner experience
  • A passion for patient-centered, proactive healthcare delivered in the home setting
