Job Description

A skilled clinician is needed to conduct home health risk assessments for Medicare Advantage plan members in Henderson County, NC, serving areas such as Hendersonville, Fletcher, and Mills River. This role offers an impactful blend of flexibility and patient-centered care, ideal for those who enjoy assessing and educating patients in their homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own work schedule
  • Minimum commitment required on a weekly or bi-weekly basis

Primary Responsibilities:

  • Perform annual in-home health risk assessments for Medicare Advantage plan members
  • Review and evaluate previous and current medical history, conditions, and medication regimens
  • Educate patients on their medical status and medication usage
  • Identify potential gaps in care and recommend actionable solutions
  • Visits do not involve prescribing medications or establishing new treatment plans

Desired Experience & Qualifications:

  • Current Nurse Practitioner license for North Carolina
  • Board Certification via AANP or ANCC
  • Graduate of an accredited MSN program
  • Active CPR (BLS) certification
  • At least 6 months experience as an NP is preferred
  • Confident in independent practice and comfortable in a home setting
  • Excellent communication and patient education skills
