Job Description

Enjoy a dynamic and flexible opportunity as a Nurse Practitioner supporting Medicare Advantage members through home health risk assessments throughout Buncombe County, NC. This per diem position provides autonomy and impact, with a focus on comprehensive annual health assessments and post-discharge visits tailored to individual patient needs in locations including Asheville, Montreat, Biltmore, and Woodfin.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule
  • Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

  • Perform in-person home health risk assessments for Medicare Advantage enrollees
  • Review prior and current medical history, chronic and acute health conditions, and medication profiles
  • Provide patient education related to ongoing conditions and prescribed medications
  • Identify care gaps and recommend solutions to improve health outcomes
  • Document findings and recommendations clearlyno requirement to prescribe medicines or create treatment plans

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board certification through AANP or ANCC
  • Master of Science in Nursing from an accredited program
  • Current BLS (CPR) certification
  • At least 6 months of current NP experience highly preferred

Compensation and Benefits:
