Job Description
Enjoy the flexibility of per diem nurse practitioner work by providing essential home health risk assessments for Medicare Advantage members. In this role, you will visit patients in their homes throughout Randolph County, delivering assessments that will have a real impact on their health and wellbeing. Each visit, averaging 45 minutes, may vary between annual health assessments and post-discharge check-ins.
Schedule
- Flexible shifts available Monday through Sunday
- Choose your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities
- Complete annual risk assessments for Medicare Advantage health plan members
- Review and evaluate patients prior and current medical histories, conditions, and medications during visits
- Educate patients on their medical conditions and proper medication use
- Identify care gaps and recommend strategies to address them
- No prescribing or development of treatment plans required
Qualifications
- Active Nurse Practitioner license issued by the state of North Carolina
- Current CPR certification (BLS required)
- Board certification through AANP or ANCC
- Graduation with an MSN from an accredited nursing program
- A minimum of 6 months experience as a nurse practitioner preferred
