Take on a vital role in improving patient care by performing in-home health risk assessments for Medicare Advantage plan members throughout Sharp County, AR, including communities such as Cherokee Village, Horseshoe Bend, and Cave City. This per diem opportunity allows for flexible shifts between Monday and Sunday, giving you the ability to create your own schedule and determine your weekly or bi-weekly commitment. Each visit typically lasts about 45 minutes and may involve annual health assessments or post-discharge evaluations.

Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage members
- Review medical history, current conditions, and medication lists
- Provide patient education on health conditions and medication management
- Identify and document care gaps; offer practical recommendations for follow-up care
- No prescribing or treatment plan development required, allowing you to focus solely on assessment and advocacy
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current CPR (BLS) certification
- Board certification via AANP or ANCC
- MSN from an accredited nursing program
- Minimum of 6 months' NP experience preferred
