Job Description
Nurse Practitioners skilled in home health are needed to conduct risk assessments for Medicare Advantage plan members living in various communities throughout Sharp County. These per diem opportunities offer significant flexibility and autonomy, allowing you to manage your caseload and create your own schedule.
Schedule:
- Flexible shifts offered every day of the week: Monday through Sunday
- Self-schedulingchoose hours that work for you
- Minimum weekly or bi-weekly hour commitments
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments (average 45 minutes per visit) in member residences across Cherokee Village, Horseshoe Bend, Cave City, and surrounding areas
- Review and evaluate previous and current medical history, underlying conditions, and medication lists
- Provide members with straightforward education on health conditions and medication regimens
- Identify gaps in care and recommend follow-up actions, supporting better patient outcomes
- Please note: No treatment planning or medication prescribing is required in this role
Qualifications:
- Active Nurse Practitioner license in Arkansas (AR)
- Current CPR certification (BLS)
- Board certified through AANP or ANCC
- Minimum of 6 months of current experience as a Nurse Practitioner is preferred
- Master of Science in Nursing (MSN) from an accredited institution
This per evaluation opportunity pays between $80-$90 per completed assessment, with additional mileage stipend and compensation for administrative duties.
