Job Description

Bring your advanced clinical expertise to a dynamic per diem opportunity conducting home health risk assessments for Medicare Advantage members throughout Jefferson County. As a Nurse Practitioner focused on home-based care, youll be pivotal in supporting patient wellness and ensuring continuity of care across Lakewood, Arvada, Westminster, and nearby communities.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Choose your own schedule
  • Minimum commitment (weekly or bi-weekly) tailored to your availability

Key Responsibilities:

  • Perform comprehensive annual risk assessments in patients homes
  • Review and evaluate prior and current medical history, existing conditions, and medications
  • Offer clear education regarding chronic illnesses, preventive healthcare, and medication usage
  • Identify potential gaps in care and make documented recommendations for closing those gaps
  • Focus assessments without the responsibility of creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Colorado
  • CPR certification (BLS) current and valid
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing institution
  • Minimum six months of recent NP experience preferred
