This per diem position offers the flexibility to craft your own schedule and support patients from the comfort of their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingchoose your own shifts
- Commitment requirement: minimum weekly or bi-weekly hours
Key Responsibilities:
- Complete comprehensive annual home health risk assessments for a Medicare Advantage health plan
- Evaluate prior and current medical history, review medical conditions, and update medication history
- Deliver patient education on medical conditions and medications
- Identify care gaps and recommend appropriate next steps for resolution
- No responsibility for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner License in Colorado
- Current CPR certification (BLS)
- Board Certified by AANP or ANCC
- Minimum of 6 months of NP experience preferred
- MSN degree from an accredited nursing program
This role is compensated at $80-$90 per evaluation, including a mileage stipend and administrative time.
