This per diem role involves conducting in-home risk assessments for Medicare Advantage plan members throughout McHenry County, IL, including areas such as Crystal Lake, Algonquin, and Lake in the Hills. Leverage your clinical expertise to support preventative care, providing crucial evaluations that guide individuals toward better health outcomes.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate each patients prior and current medical history, medical conditions, and medication usage during in-home visits
- Educate patients about their chronic conditions and prescribed medications to encourage informed health decisions
- Identify care gaps and make recommendations, contributing to comprehensive care coordination
- Assessments may vary in nature, including annual health assessments or post-discharge visits
- Each in-home evaluation will typically last about 45 minutes
- No treatment plans or medication prescriptions are required
Qualifications:
- Active Nurse Practitioner license in Illinois
- Board Certification via AANP or ANCC
- Completion of an MSN from an accredited nursing program
- Current BLS (CPR) certification
- Preferred: Minimum of 6 months recent Nurse Practitioner experience
