Job Description

As a skilled Nurse Practitioner with experience in home health, youll play an integral role in assessing and educating Medicare Advantage members in McHenry County, Illinois. This per diem opportunity allows you to conduct home health risk assessments in a rewarding setting that values flexibility and autonomy. Each assessment lasts about 45 minutes and may include annual health evaluations or post-discharge visits within diverse communities such as Crystal Lake, Algonquin, and Lake in the Hills.

Schedule:

  • Choose your own shifts, available any day Monday through Sunday
  • Flexible scheduling to suit your availability
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Evaluate prior and current medical history, medical conditions, and medication history
  • Provide clear, patient-centered education on medical conditions and medications
  • Identify gaps in care, offering actionable recommendations to improve health outcomes
  • All visits are strictly assessment-focusedno treatment plan development or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in Illinois
  • Board Certification through AANP or ANCC
  • MSN from an accredited nursing program
  • Current CPR (BLS) certification
  • At least 6 months of recent NP experience preferred
