Make a direct impact on community health by conducting vital home health risk assessments for Medicare Advantage members across Clark County, AR. In this per diem nurse practitioner opportunity, you'll visit patients in their homeslocations may include Arkadelphia, Gurdon, Amity, and surrounding areas. Each assessment takes approximately 45 minutes, ranging from annual health evaluations to post-discharge visits, allowing for meaningful contributions to patient well-being without the need for prescribing or creating treatment plans.

Key Responsibilities:

• Perform annual and post-discharge risk assessments for Medicare Advantage members
• Review and evaluate patients medical histories, current conditions, and medications
• Provide targeted education regarding health management and medication adherence
• Identify care gaps, documenting findings and making effective recommendations
• Maintain clear records while supporting collaborative patient care

Qualifications:

• Nurse Practitioner license in Arkansas
• Active BLS/CPR certification
• Board-certified through AANP or ANCC
• At least 6 months of recent nurse practitioner experience preferred
• MSN from an accredited nursing program
