Job Description

Deliver meaningful care and support to Medicare Advantage plan members by conducting home health risk assessments across Lee County, IA in a flexible, per diem capacity. This Nurse Practitioner opportunity offers autonomy, highly flexible scheduling, and the ability to make a genuine impact on patient well-being in their home setting.

Schedule:

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage members
  • Review and evaluate patient history, medical conditions, and current medications
  • Educate patients on their health conditions and prescribed medications
  • Identify potential gaps in care and provide recommendations to optimize patient outcomes
  • Focus on assessment and educationno treatment planning or prescription management required

Desired Qualifications:

  • Active Nurse Practitioner license in IA
  • Graduate of an accredited MSN program
  • Board-certified as an NP by AANP or ANCC
  • Current CPR (BLS) certification
  • Minimum of 6 months NP experience preferred
  • Excellent interpersonal, documentation, and assessment skills

This role pays $80-$90 per completed evaluation, including additional mileage stipend and compensation for administrative time.
