Job Description

Make a meaningful difference in patient care by providing in-home health risk assessments for Medicare Advantage plan members throughout Grant County. As a Nurse Practitioner specializing in home health, youll enjoy an engaging, patient-centered role that prioritizes flexibility and your professional expertise.

Key Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage members
  • Review patients medical histories, current medical conditions, and medication use
  • Educate patients on chronic conditions, preventive care, and medication management
  • Highlight care gaps and recommend actionable steps for improved health outcomes
  • Focus on assessment and educationno development of treatment plans or medication prescribing is needed

Qualifications:

  • Active Nurse Practitioner license in New Mexico
  • Current CPR certification (BLS required)
  • Board certified through AANP or ANCC
  • At least 6 months of recent NP experience is preferred
  • Master of Science in Nursing (MSN) from an accredited institution

Youll visit patients in various locations within Grant County, such as Roswell, Bayard-Santa Rita, Santa Clara, Tyrone, and Hurley. Each home health risk assessment is allotted approximately 45 minutes, and assignments may include annual wellness visits or post-discharge follow-up assessments.

This per diem position provides a highly competitive rate of $80-$90 per completed evaluation, plus additional compensation for mileage and time spent on administrative tasks.
