Schedule:

Role Overview:

  • Travel to patient homes in Henderson County, including areas such as Hendersonville, Fletcher, and Mills River
  • Conduct annual or post-discharge health assessments, typically lasting about 45 minutes per visit
  • Evaluate patients medical histories, current conditions, and medication usage
  • Deliver patient education regarding medical conditions and medications
  • Identify care gaps and make recommendations to optimize patient outcomes
  • No responsibility for prescribing medications or creating treatment plans

Desired Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current CPR (BLS) certification
  • Board certification via AANP or ANCC
  • Minimum of six months of NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
