Deliver comprehensive care by performing home health risk assessments for Medicare Advantage members in the heart of Henderson County. Make a positive impact as a Nurse Practitioner through per diem work, providing vital services in locations such as Hendersonville, Fletcher, and Mills River. This role gives you the autonomy and flexibility to create a schedule that fits your lifestyle, supporting patients from post-discharge assessments to annual health evaluations.
Schedule:
- Flexible shifts available seven days a week
- Set your own work hourschoose shifts that match your availability
- Minimum commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Complete detailed health risk assessments (each lasting approximately 45 minutes) in patients homes
- Review and evaluate patients past and present medical histories, conditions, and medication usage
- Offer education on diagnoses and guide patients about their medications
- Detect gaps in care, communicating actionable recommendations
- No need to prescribe or create treatment plansfocus is strictly on evaluation and education
Qualifications:
- Valid Nurse Practitioner license in North Carolina
- Active CPR (BLS) certification
- Board certified by AANP or ANCC
- Six months of recent experience working as a Nurse Practitioner preferred
- Master of Science in Nursing from an accredited institution
This position offers competitive per-evaluation earnings ranging from $80 to $90, along with a mileage stipend and compensation for administrative time.
